Test Requisition Form

The GenoPredicta™ Test must be ordered by a licensed Medical Provider through a Test Requisition Form (TRF).

The combined TRF and patient consent forms are available here and are also included with the GenoPredicta™ kit.

A completed TRF must accompany all samples/kits. Completed consent forms can also be emailed or faxed to (857) 855-5272. Licensed medical professionals may also submit TRFs online via DocuSign.

Testing services are currently available in all U.S. states except New York. We are actively working to bring access to this location. To be notified when testing becomes available, please contact us.
